746.3805(b)(1) - Administering Medication - Original Container with Child's Name, Date

it was observed in the infant 1 room there was teething medication unlabelled, and intented for the use of all infants. The medication was removed during the inspection.

746.3703(c) - Not Under the Influence of Alcohol or Controlled Substance

There was sufficient evidence found during the investigation to show that one staff member was in direct care of children after consuming alcohol.

746.2911 - Adequate Lighting During Naptime

One classroom did not have adequate lighting during nap time. This was corrected during the inspection when a light was turned on.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

A classroom was observed with 7 infants aged 10-16 months during nap time. There was one caregiver present bringing the classrooom out of the required child/caregiver ratio of 5:1.

746.2426(a)(2) - Infant May Not Sleep in a Restrictive Device - Remove the infant from the device and place the infant in a crib as soon as possible

Two infants were observed sleeping in restrictive devices. The infants were moved to bare cribs during the inspection.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

Caregiver did not do a face to name count upon reentering the building with the group of children. Caregiver left a child outside unattended for over 20 minutes. Caregivers kept eyes on children for transition into building from playground.

746.1201(4) - AP Responsibilities of Employees and Caregivers -Ensure No Child Abused, Neglected, or Exploited

Based on the DFPS investigation there is a violation of this standard. A child was left outside, unattended, in the heat of summer at the operation for over 20 minutes.
