A cargiver was observed supervising a child in the bathroom from the doorway of their classroom and would have been unable to intervene without leaving their class unsupervised if the child needed help.

746.3701(1) - Safety - Electrical Outlets Covered

There was an outlet in the afterschool room that did not have a cover. This was corrected during the inspection when an outlet cover was placed in the outlet

A staff member with a conditional background check was observed supervising a group of children alone while the main caregiver was in the restroom. This was corrected during the inspection when the main caregiver returned from the restroom.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

A caregiver stated that they had 12 children in their class when they had 13.

746.3805(b)(1) - Administering Medication - Original Container with Child's Name, Date

A child had a bottle of Tylenol that did not have the child's name and the date the parent brought it on it.

746.3417(3) - Children Handwashing After Toileting or Diapering

A caregiver did not wash a child's hands after the child had their diaper changed. This wsa corrected during the inspection when the caregiver was reminded about handwashing and the caregiver washed the child's hands.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

A caregiver who was outside with their class on the playground sent a child inside to use the bathroom alone. This was corrected during the inspection when the director was informed and the child was returned to their class.

746.2419(8) - Requirements for Feeding Infants - Other Use of Handwashing Sink

A caregiver washed their hands in a sink that had bottles in it and was used for washing bottles.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

The afterschool room had a bottle of Clorox wipes sitting on a desk within reach of children, and the pre-k room had a bottle of Clorox and a bottle of hand sanitizer sitting on the desk within reach of children. This was corrected during the inspection when the Clorox bottles and hand sanitizer were moved out of reach.

746.1301(a)(3)(A) - Training Requirements for Employees, Caregivers, and Directors - Pediatric First Aid with Rescue Breathing

The director and three caregivers had expired first aid and CPR, and two caregivers did not have proof of first aid and CPR.

746.3415(4) - Employee Handwashing After Diapering a Child

A caregiver did not wash their hands after diapering a child. This was corrected during the inspection when the caregiver was reminded about handwashing and the caregiver washed their hands.

745.621(a)(2) - AP Initial background checks submitted - At the time you hire someone

A caregiver did not have background check submitted. This was corrected during the inspection when the caregiver left the operation and the director submitted a background check for them.

746.2431(4) - Infant Daily Report - Infant's Mood

Infant caregivers were not adding mood and activities to Brightwheel.

746.3809 - Medication No Longer Used

One child had an EpiPen that expired in May 2024.

746.303(b) - Report Number of Employees

A review of the Employees Who Left Employment in 2024 data revealed the center did not report the number of employees who left employment in 2024 by January 15, 2025, as required.
