Based on information gathered during the investigation, the operation exceeded the maximum group size for two or more caregivers and exceeded the maximum group size for one caregiver on the day of the incident.

Based on information gathered during the investigation, one caregiver was completing incident reports, while the other two caregivers were cleaning during the time the child was last seen.

Based on information gathered, the operation did create an incident report following the incident.

Based on information gathered through the investigation, it is determined that a child in care was missing when parents arrived for pick up. Multiple caregivers were unaware of the location of the child for over 15 minutes.

Based on information gathered through external documents and interviews conducted, it was determined that a child with special needs was smacked on the hand by a caregiver while in care.

During the course of the investigation, it was determined that a caregiver has a history of yelling at children in care.

744.1203(4) - Provide care consistent with child's habits, interests, strengths, special needs, including any special supervision needs

Based on information gathered through the course of the DFPS investigation, it was determined that appropriate and consistent care was not provided for a child with special needs.
