The operation has not provided the Childcare Regulations with the required paperwork for a designated site director.

Based on information gathered through the course of the investigation, it was determined that a caregiver did not exercise good judgment by allowing a student to sit in their lap and engage in hugging in a manner that is inappropriate. The investigation also found that the caregiver was carrying children on his hip and holding them in his arms in a manner inconsistent with appropriate caregiving practices.

Based on information gathered during the investigation, it was determined that a child in care was injured on a trampoline during an off-site field trip.

The operation has not submitted the required educational information needed for a caregiver that was submitted site director to submit on March 31, 2025.

The former site director did not use good judgment by starting a group text message with three children at the operation while she was employed. During that time, the site director shared personal information and other adults' private information with the children.

The operation did not notify one family involved in the investigation of the incident. The other child's family did not sign the incident report that was sent via email.
