During the inspection a caregiver in the gym sent a child to the cafeteria and did not notify the other caregivers she was being sent there. The caregiver did not ensure caregiver to caregiver visual contact of the child was conducted.

Based on information gathered during the investigation, it was determined that a caregiver who is 18 years old and attends high school but has not graduated was left alone with sole responsibility for a group of children.

Based on information gathered during the investigation, it was determined that the operation did not notify Licensing regarding the appointment of a new site director.

Based on information gathered during the investigation, it was determined that the permit holder is not adhering to the applicable minimum standards regarding child-caregiver ratio, required notifications, caregiver qualifications, and the Texas Administrative Code regarding Background checks.
