746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

On 5/7/2026, the DFPS investigator observed a child unsupervised by their caregiver for approximately three minutes.

746.3501(1) - Diaper Changing Steps- Prompt

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with multiple parents, it was determined that their children's diapers are not being promptly changed.

746.307(c) - Parental Notification of Less Serious Injuries

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with multiple parents and review of incident reports, it was determined that parents have not been receiving same day communication regarding minor injuries that their children received while in care.

746.2431 - Infant Daily Report - Share with Parent

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with caregivers, it was determined that the caregivers are not consistently filling out and sharing the infant daily reports with the parents which includes information about diaper changes, food/bottles consumed, times the infant slept, infant's general mood for the day and summary of the infant's activities.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

During the inspection a caregiver who had an inactive background check status was present and caring for children. This was corrected during the inspection when the person was sent home.

746.3503(d) - Diaper Changing Equipment - Hand-washing Sink in Diaper Changing Area

During the inspection the diaper changing tables in the Infant and Waddler classrooms were placed next to hand washing sinks which were not operating. This was corrected during the inspection in the Waddler classroom when the diaper changing table was moved next to the restroom.

746.3407(13) - Using, Storing and Disposing of Hazardous Materials

During the inspection there were hazardous materials stored in an unlocked cabinet, within the reach of children, located in the preschool restroom. This was corrected during the inspection when the cabinet was safeguarded by staff.

746.1309(e) - Documented Annual Training- Required Content

The training records for two caregivers did not have training in storing and handling of hazardous materials, and responding to food allergies.

746.603(a)(3) - Children's Records - Health Statement

Two out of ten children's records did not contain physician's health statements.
