746.613(a) - Required Immunizations

Immunizations were not up to date after an immunization audit completed by the Department of State Health Services.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Information obtained throughout the investigation shows that caregivers failed to supervise children when two children pulled each other's pants down while playing outside.

746.1107(2) - Additional Minimum Qualifications for Caregivers - HS Diploma, Equivalent, Certification of Completion

An employee was performing caregiver duties without meeting caregiver qualifications. This was corrected when the caregiver stopped caregiver duties.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

One infant was sitting in a highchair without the safety straps fastened. The safety straps had been removed from the chair. This was corrected when a caregiver removed the infant from the chair.

Information obtained throughout the investigation shows that naptime mats in the toddler room did not have sufficient space between each naptime mat.

746.1013(a)(1) - Director's Absence - Caregiver in Charge Knows in Charge

The person in charge when the director was not present was unaware they were in charge. Other caregivers asked did not know who was in charge or gave the wrong person's name. This was corrected at inspection when the director called and informed the person in charge that they were in charge and later when the director arrived at the operation.

746.3427 - Pest Extermination

It was found during an investigation that while the operation had regular professional pest control treatment, the operation staff had also been treating the outdoor area with a mosquito outdoor fogger spray without being certified to do so.

746.1205(a)(5) - Responsibilities of Caregivers - Intervene to ensure each child's safety

The operation was unable to intervene when a child ran out of the classroom and locked themself in the director's office on two different occasions. During both occasions the operation sent another child through the director's office window to assist in unlocking the door. On the second occasion the child who locked themself in the office began repeatedly hitting the child while they tried to unlock the door.

746.2207(c)(3) - Screen Time Activities - Not Exceed One Hour Daily

Information obtained throughout the investigation shows that a caregiver used screen time activities for more than one hour per day.
