746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

Two caregivers did not know the number of children they were responsible for when asked twice. This was corrected when the caregivers counted the number of children and gave the correct number.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

An infant under the age of 12 months was observed asleep in a play yard with extra bedding. The manufacture instructions of the extra bedding stated, "Never use in a crib or play yard". The extra bedding was removed at inspection.

746.2401(6) - Basic Requirements for Infants - Free from Choking Objects That May Cause Choking in Infants or Toddlers

An infant under the age of 12 months was observed asleep in a play yard with a pacifier string attached to their clothing. The pacifier string was removed during inspection.

746.2411(2)(B) - Safety Requirements for Play Yards - Snug Fitting Sheets

An infant under the age of 12 months was observed asleep in a play yard with a loose-fitting sheet. The infant was removed from the play yard at inspection.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

While reviewing video footage for another investigation, a caregiver used bad judgement when attempting to move the diaper changing mat while an infant is on it. The caregiver is observed sitting on the floor with the diaper changing mat in front of them, picking the diaper changing mat up while an infant is on it and dropping it. The diaper changing mat was padded and landed up correctly however, the caregiver exhibited bad judgement when attempting to move it this way.

746.3317(1) - Food Service and Preparation Requirement - Sanitize Equipment

While reviewing video footage for another investigation, a caregiver was observed preparing an infant?s food using a utensil to try the food and then using the same utensil to prepare the food on the plate. The caregiver was observed eating food off of a plate and then giving the infant the remainder of the food on the plate while using the same utensil the caregiver ate off of.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

While reviewing video footage for another investigation, a caregiver is observed exiting the classroom on multiple occasions in short durations while the infants are eating their food.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

While reviewing video footage for another investigation, a caregiver was observed using her cellphone on multiple occasions while caring for the infants in the classroom.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

While reviewing video footage for another investigation, an infant was observed sleeping in an exerSaucer and an infant was observed sleeping in a bouncer.

746.3415(4) - Employee Handwashing After Diapering a Child

While reviewing video footage for another investigation, a caregiver was not observed washing her own hands between each diaper change.

746.3505(e) - Diapering Procedures - Sanitizing Diaper Changing Area

While reviewing video footage for another investigation, a caregiver was observed not disinfecting the diaper changing mat between each use.

746.3421(b) - Infant Handwashing - Raised to Sink

While reviewing video footage for another investigation, a caregiver was not observed washing any of the infant?s hand throughout the day. The caregiver is only observed wiping the children?s hands and faces with wipes.

746.3415(1) - Employee Handwashing Before Eating or Handling Food

While reviewing video footage for another investigation, a caregiver was not observed washing her own hands before preparing meals.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

While reviewing video footage for another investigation, a caregiver was observed inappropriately grabbing children by their arm when picking them up and moving them around the classroom.

746.1003(4) - Director Responsibilities - Employees Supervised

During a DFPS investigation, there was sufficient evidence to support that the director was not appropriately supervising caregivers when a caregiver was observed on video walking out of a classroom leaving infants unsupervised multiple times.

746.1201(4) - AP Responsibilities of Employees and Caregivers -Ensure No Child Abused, Neglected, or Exploited

During a DFPS investigation, there was sufficient evidence to support that a caregiver abused and neglected children in care.

746.3701 - Safety - Areas Free From Hazards

During a DFPS investigation, there was sufficient evidence to support that a caregiver left their purse on the floor of a classroom and accessible to infants.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

During a DFPS investigation, there was sufficient evidence to support that a caregiver dropped an empty bouncer seat on the ground next to two infants on the ground. The bouncer seat did not hit the infants, however the caregiver exhibited bad judgement when attempting to move it this way. During a DFPS investigation, there was sufficient evidence to support that a caregiver did not demonstrate good judgement when the caregiver sprayed Lysol in the close proximity of infants. During a DFPS investigation, there was sufficient evidence to support that a caregiver did not use good judgement when they used their sleeve to wipe an infant's nose twice. Additionally, the caregiver did not wash hands after doing so.

746.201(7) - Operation Open to Parents At All Times

During a DFPS investigation, there was sufficient evidence to support that parents are only allowed in the doorway of the operation, they are not allowed in the classrooms.

746.2805(8) - Prohibited Punishments - Restrictive device for time out

Infants are being placed in a restrictive device as a form of discipline.

746.1203(3)(C) - Janitorial duties

While caring for infants a caregiver was observed washing dishes, sweeping, vacuuming and spraying disinfectant in the room.

746.201(2) - Written Personnel Policies

A caregiver failed to follow the operations smoke free and drug free section the personnel policies.

746.2805(1) - Prohibited Punishments - Corporal Punishment

During a DFPS investigation, there was sufficient evidence to support that a caregiver was roughly moving infants when they placed them in a highchair and when they removed them from bouncer seats or exersaucers.

746.3317 - Food Service and Preparation Requirement - Safe and Sanitary

Video footage showed a caregiver spraying Lysol around themselves while shaking a bottle for an infant.

746.305(a)(6) - Report Situation Placing Children at Risk

During a DFPS investigation, there was sufficient evidence to support that it was not reported that a caregiver was using prohibited discipline and guidance on infants after the director observed video of the classroom.
