An Authorization to Administer Medication form for a child's Bendaryl topical lotion stated to apply, "As needed," was not in the original container, and the length of the authorization exceeded the manufacturer's seven-day limit. A written medication authorization should include the child's name and birthdate, name of medication, administration instructions, medication intervals and length of the authorization dated and signed by the parent is on file.
