Staff A has been observed to be yelling at children and yelling close to children's faces. She told children she doesn't want them in her classroom and that "they don't belong there." During the collection of attendance records, staff admitted that records had been pre-dated and did not match the actual days of attendance. Staff spent time during the visit attempting to correct the documents in question. Documentation shows that on 05/21/2025, the sign in sheets shows 9 children in the classroom all day, although the count says 8 children:2 teachers. In a review of attendance, the number of children in a group exceeded the maximum number allowed in the Infant 1 and Todd 1 rooms multiple times, including but not limited to: On 05/19/2025 the Infant 1 classroom had up to 11 children in a group of infants and toddlers for up to two hours. On 05/21/2025 the Infant 1 room was over max group size with 15 children for up to 30 minutes. On 05/22/2025, the Infant 1 room was over max group size with 14 children for up to 30 minutes . A review of attendance shows that staff-to-child ratio was not met multiple times. On 05/19/2025, the Infant 1 room was over ratio up to two hours when 1 teacher was alone with 5 children and then 2 teachers were in ratio with up to 11 children. On 05/21/2025 the Infant 1 room was over ratio for up to 20 minutes when 2 teachers were in ratio with 15 children and again on 05/22/2025, when 2 teachers were in ratio with 11 children for 20 minutes.
