There were toy pieces on visibly dirty floors of multiple bathrooms. Multiple books in the early preschool classroom were ripped, crumpled and unable to be used. In an unlocked cot storage cabinet in the early preschool room, containers of paints and food coloring had fallen off a shelf and were left in a pile. Center policy says when they first open for the day and attendance is low, and at the end of the day, as children are picked up, children may be placed in rooms other than their assigned classroom. Center policy states staff will communicate with parents when their child is moved to the next classroom. However, children are often moved to classrooms that are not their assigned classrooms at all hours of the day. Policy states that it is unsafe for infants and toddlers to have older children in their rooms but staff place older children with behavior issues in younger children's rooms. The premises was not clean and in good repair. For example, floor tiles were broken in multiple bathrooms and unable to be sanitized. Review of attendance showed maximum group size for groups that include infants and toddlers was exceeded on July 31 in the infant 2 room for 15 minutes within a 1.5 hour period starting at 7:31AM when Staff G and Staff J were with nine children, including eight who were not yet 1.5-years-old and one who was at least 1.5-years-old. On August 7 maximum group size was exceeded for at least 10 minutes starting at 8:21AM when Staff A, Staff G and Staff K were signed in to care for nine infants and toddlers when only eight are allowed.
