The center self-reported that on March 10, 2026, after returning from the playground, two child care workers failed to closely supervise a 1.5-year-old child during a transition from the playground to the classroom. The child was left in the parking lot alone while the group went inside. The child was found by a parent who informed the center that a child was outside alone.
