On 5/21/25 staff did not check the name on the bottle of breast milk and fed it to the incorrect child. Both sets of parents were called. Neither set of parents had an issue with the error. Per discussion with the Center Director, on 9/16/24 on child in n the 4 K room was not within sight and sound supervision of a child care worker when the worker was unaware of the child in the bathroom and took another child outside to join the group.
