   Based on a self report, on June 19, 2025, a child care worker failed To Implement The Center's Child Tracking Procedure of Doing A Head Count When The Children Were transitioning to the outdoor play space and when they got to their destination resulting in the worker leaving a 4.5-year-old child in the classroom. A child care worker arrived just as the class was leaving and found the child on their cot. Rule 251.055(1)(f) (View official inspection document).
