The center was issued a non-compliance statement on 5/28/26 for rule violations at the center. A correction plan was received by the Licensing Specialist on 5/29/26 but it was not acceptable in one area. The correction plan was sent back to the licensee stating a revision was required. On 6/22/26, the Licensing Specialist emailed the licensee stating a revised correction plan had not been returned to the Department and was due by 6/26/26. Supervision - Opening & Closing Hours (251.055(1)(c))

Rule states in pertinent part 'Assistant child care teachers who are at least 18 years of age and have completed the training required for the position may provide sole supervision to a group of children for opening and closing hours, not to exceed the first 2 hours and last 2 hours of operation.' Contrary to rule, an 18 year old employee (Staff D) was providing sole supervision during the first and last 2 hours of operation, but was not qualified to do so, as she had not completed the required training for an Assistant teacher. Rule states "A child care worker shall record any evidence of unusual bruises, contusions, lacerations, or burns seen on a child, regardless of whether received while in the care of the center". On this date, this Licensor noticed a young child with a long, reddened scratch under his/her right eye.
