Based upon self report, on April 17, 2026, a child care worker failed to ensure that a child was fed the correct breastmilk. A child in care consumed two bottles of the wrong breastmilk. It was discovered when the mother picked up the child from care and determined the bottles in the child's bag were not her child's and the child's breastmilk remained in the fridge. Based upon investigation, on November 22, 2024, a child care worker failed to adhere to the center's child tracking procedure when they failed to conduct a name-to-face, count the children and do a scan of the room prior to leaving the Ladybug Room, resulting in a 6 month old child being left alone in the Ladybug Room nap area for approximately 4-5 minutes. Based upon investigation, on November 22, 2024 another infant was not signed out of the Ladybug Room when a child care worker removed him from the care of the assigned child care worker and took him to the office to give him a bottle.
