The center self-reported that on 11/4/25 a child was not closely supervised by child care workers who were within sight and sound of the child to guide the child's behavior and activities, prevent harm and assure safety when the child wandered away from his group as they were undressing in the hallway having come inside from playing out on the playground. Child Tracking Procedure (251.055(1)(f))

On November 4, 2025, the center's tracking procedure failed when staff did not know the whereabouts of one child for approximately 2 minutes when the staff failed to count children as they undressed from being outside and transitioned back into their classroom.
