During a staff file review, one employee was missing a few items from his/her file, resulting in an incomplete record. The items included a staff health report form, High School diploma, education qualifications and documentation of a completed caregiver background check. *It should be noted a background check was processed as required, but the file didn't contain a paper copy or documentation, as required. Cleanliness Of Furnishings, Toys, Equipment (251.07(3)(i))

Rule states states "The premises shall be well drained, free from litter, clean and in good repair." During this licensing visit, the premises appeared unclean. Waste containers had what appeared to be dried milk splashes on the top surface of the containers and then dripping down the sides. Bathroom doors, center walls, trim, and light switches had large areas of dirty smudges across them. And a couple area rugs were dirty or stained.
