On May 23, 2025, the indoor premises failed to be free of hazards when a vape cartridge fell out of a child care workers pocket onto the floor. The cartridge was then picked up by a 15-month-old child, held, and then placed in their mouth. The child had the cartridge for approximately 29 seconds before being noticed by the worker and taking it away.
