Based on a complaint investigation it was determined that child care workers failed to give a 4-year-old child individual attention. The child care workers failed to allow the child to go to the bathroom at nap time, stating they already went. A parent had to get a doctors note in order to allow their child to use the bathroom at nap time. Child care workers wouldn't allow a 4-year-old child their security item. Based on a complaint investigation child care workers failed to implement the centers child guidance policy: When A Child Is Crying, Fussy Or Distraught, Staff Will Work To Calm And Comfort The Child, In Ways That Are Appropriate For The Child's Age And Personal Disposition. This May Include Stroking, Cuddling, Rocking; Offering A Drink; Acknowledging The Child's Fear, Or Separation Sadness, Or Conflict. A 4-year-old child was crying and upset to the point of hyperventilating. Worker said in a firm voice "stop crying, stop it, cut it out, your all done crying your mom will be here later." Policy - Child Guidance (251.07(2)(b))

Based on a complaint investigation and observation of the center it was determined a child care worker failed to implement the centers child guidance policy which states staff provide positive guidance for the purpose of helping children develop self-control, self-esteem and respect for the rights of others. The child care worker said in a loud, harsh tone towards children "are you pooping", "No", and "We don't do that". Based on observation of the infant classroom the nap room failed to have exits and passageways unobstructed. The center self reported that on 11-10-25, child care workers failed to supervise four 2-year-old children when the director witnessed the children leave the gated play space. The child care workers were not aware the child left the play space. On 11-12-25, child care workers failed to supervise a 2-year-old child when they left the child alone in the classroom, for approximately 15 minutes, while the rest of the group went outside to the play space.
