The baby gate separating the infant room and other classrooms and to the kitchen was broken. The door leading to the kitchen area was also unlatched at the the time of the visit, so a child could have exited the classroom and entered the kitchen where there was a kitchen knife on the edge of the counter and within reach of children. There were prickly weeds observed in the outdoor play space. There was a loose fence board in the outdoor play space which had a nail protruding from it.
