Supervision Of Children (251.055(1)(a))

All children were not supervised by a staff member within sight and sound of the children when, on October 28, 2025, a 2-year-old child opened a gate and left the playground, walked across the parking lot, and was found in the street in front of the center. Child Tracking Procedure (251.055(1)(f))

The center's child tracking procedure failed when, on October 28, 2025, a 2-year-old child opened a gate and left the playground, walked across the parking lot, and was found in the street in front of the center. The child's whereabouts were unknown to the assigned caregivers for 2 to 4 minutes until the child was returned to the group. This incident was self-reported by the center.
