The licensee failed to report to the department within the required 24 hour time frame an incident or accident that occurred on 6/5/25 that resulted in professional medical evaluation. The licensee became aware on 6/5/25 that a child was taken for professional medical evaluation after falling off a teeter totter. The child was determined to have a broken arm.
