Each child was not supervised by a child care worker who was within the sight and sound of the children to guide behaviors and activities, prevent harm and ensure safety when on February 11 a 3-year-old child was unsupervised for 2 to 4 minutes after he was found by his teacher unattended in the classroom while the class was outside. On December 17 and 18, 2024, while working as an assistant teacher, Staff A spanked their own child while the child was in care at the center. On January 16, 2025 while working as a teacher, Staff B attempted to spank their own child while the child was in care at the center. These prohibited actions were self-reported by the center.
