Based on a complaint investigation it was discovered a staff member within the 1-2 year old room used inappropriate child guidance on 5 children in care. Child Guidance - Prohibited Actions (251.07(2)(e))

Based on a complaint investigation a child care worker used inappropriate child guidance on 1-2-year-old children in care. The worker flicked the bottom of children's feet to make sure the children kept their shoes on and swatted children's hands when they would hit other children or touch their diaper area during diaper changes. Based on observation of the center on 10/14/25 their were a number of missing outlet covers throughout the center. Based on observation of the center it failed to be in good repair. It was observed that Dry wall is exposed in multiple classrooms areas.
