Based upon self report on September 13, 2024, the center did not follow the current policy for tracking during a transitional period resulting in a 2 year old child being found in the teddy bear classroom after the class went outside. Based upon observation, the staff handwashing sink was not maintained in a clean manor when a mop head was resting over the neck of the handwashing sink for staff between Teddy Bear and Butterfly room. Based upon observation, the door in the butterflies classroom, had wooden finish chipped off resulting in sharp protruding pieces accessible to children in care.
