Based upon staff interview, on April 20, 2026, a child care worker failed to implement the name to face procedure when coming in from outside resulting in a two-year-old child being left unattended in the outdoor play space. The worker was calling the children's names when another child needed attention. The worker stated the child's name was called; however, the child's name was not checked.
