On 05/28/2025, Child 1 was without site and sound supervision when they exited the classroom. Child Tracking Procedure (251.055(1)(f))

On 05/28/2025, Child 1 exited the classroom without staff knowledge. Staff did not implement the center's child tracking procedure to ensure all staff know children's whereabouts at all times. On 06/03/2025, the center's child tracking procedure failed when Child 1 was left in the outdoor play space while the rest of the group transitioned inside.
