Factual basis: Three staff members have been listed as trainee for longer than 12 months and have not yet completed Fundamentals or a Child Development course to meet requirements. Corrective action: The staff members should enroll in the next available fundamentals course or a college course meeting requirements. Provide documentation of enrollment for each staff member to sarah.crosier@vermont.gov to demonstrate compliance. Completion of the coursework will be assessed at the next compliance visit. Factual basis: In a review of 9 child files, 5 were missing documentation of a well child exam. Corrective action: Obtain documentation of well child visits for each enrolled child. Create a plan to ensure that this document is obtained for each future enrolled child no later than 45 days after enrolling. Notify sarah.crosier@vermont.gov when all items are complete. Factual basis: Fifty percent of staff had items documenting completion of training that had not yet been sent to Northern Lights for entry into BFIS. This is a repeat violation. Corrective action: Immediately ensure that staff send all existing documentation to Northern Lights through Direct Send. Create a plan to ensure that each staff member submits documents in a manner that maintains an up to date Bright Futures account. Submit that plan to sarah.crosier@vermont.gov for review. Factual basis: The documentation confirming the Program Director's qualifications for the position are not within the BFIS account. This was identified as a requirement to be met during the compliance visit conducted on 07/02/2024. The BFIS accounts for six (6) staff persons contained expired CPR certification and the current CPR certification documentation was confirmed on site. Corrective action: Submit the required qualification documentation to Northern Lights (NL@CCV) for uploading to each individual's Quality and Credential account by 09/01/2025. Send confimation to Licensing Field Specialist Crawford when the documentation is confirmed to be present in each person's account. Develop a plan that establishes who and how documentation will be submitted in a timely fashion by 09/15/2025. Factual basis: In a review of documentation of required monthly evacuation dates only seven (7) of the required seventeen (17) due between January 2024 and May 20205 were recorded. The date of one (1) emergency evacuation drill (fire drills)was recorded for March, April, May, November of 2024 and one for April of 2025. Two (2) drills were conducted for July 2024. Corrective action: Develop a plan that identifies who is responsible for scheduling, overseeing, and documenting the monthly evacuation drills. Regulation 2.3.9.3: The licensee shall ensure that all staff have met ongoing background check requirements by submitting at least once every five (5) years a Records Check Authorization form to the Division and as required shall submit to fingerprinting.

Factual basis: Staff person A's five (5) year renewal date for the background clearance process was 10/18/2024. As of the date of this visit, this individual had not submitted the required record check authorization form. Corrective action: Ensure that Staff Person A completes the required fingprinting process. The program and the staff person were notified of this requirement following the processing of the record check form on 07/02/2025.
