Regulation 3.6.2.3: At least one (1) drill every 365 days shall be conducted while children are sleeping or resting.

Factual basis: No evacuation drill was conducted during rest time in the last 12 months. Provider stated she believed it was ok to look at calendar year and one had been done in March 2025. Corrective action: Complete an evacuation drill during June that occurs during rest time. Notify sarah.crosier@vermont.gov when this is complete. Regulation 7.4.4: All staff, with the exception of substitutes, shall complete fifteen (15) clock hours of annual professional development activities as required in the rule 7.4.2 of these regulations.

Factual basis: The provider has completed and documented 6 of the required 15 professional development hours for the review period 7/4/2023-7/4/2024. Corrective action: By 7/4/2025, provider should complete 15 hours for the 7/4/24-7/4/2025 period. There are currently 2 hours documented in BFIS. Additionally, by 9/1//2025 she should complete an additional 9 hours of professional development to make up the missing hours for this review period. Factual basis: IRC has not been completed within the previous year. No immediate concerns for chipping or flaking paint were noted. Regulation 5.6.6: The FCCP shall ensure that all prescription medication is in its original container, properly labeled for the child, and administered prior to the expiration date on the container.

Factual basis: A review of medication administration records found that a medication had been administered after the expiration date on the medication. The expired medication remained on site. The provider stated that she did not realize it was expired at the time and expressed concern that not having any rescue medication on site when a child needed it would be worse than having an expired medication until the family was able to get another one. Corrective action: Remove the expired medication from the program and request a new one from parents with current documentation.
