Factual basis: No evacuation drill was conducted during rest time in the last 12 months. Provider stated she believed it was ok to look at calendar year and one had been done in March 2025. Factual basis: A review of medication administration records found that a medication had been administered after the expiration date on the medication. The expired medication remained on site. The provider stated that she did not realize it was expired at the time and expressed concern that not having any rescue medication on site when a child needed it would be worse than having an expired medication until the family was able to get another one. Corrective action: Remove the expired medication from the program and request a new one from parents with current documentation.
