   Factual basis: Six staff members who have been employed for more than 30 days do not have documentation of completing the Vermont Orientation Training in BFIS, it was not presented on site, and it was not sent to licensing during the week following the site visit. Two of these individuals were present and in ratio during the licensing visit. Corrective action: Ensure that all staff complete the Vermont Orientation Training or an approved on site orientation training prior to being left alone with children or counting in ratio. Corrective action: Develop a plan to ensure that each staff member and substitute completes the required number of professional development hours annually and submits them to Northern Lights for entry into BFIS.    Factual basis: Five staff members employed longer than 30 days do not have documentation of completing Protecting Vermont's Children mandated reporter training in their BFIS accounts. Documentation was not provided on site, nor was any sent to licensing in the week following the site visit. Two of the staff were in ratio during the licensing visit. Corrective action: Ensure that all staff complete this required training prior to working with children. Send copies of the completion certificates for current staff and develop a plan to ensure that all new staff complete this training prior to working with children and submit completion certificates to Northern Lights for entry into BFIS. This includes both ongoing staff who have not renewed their certification prior to the expiration date of their previous certification and new employees who have not completed the training within 3 months of their start date. Corrective action: Immediately ensure that all current staff complete this training and submit completion documentation to sarah.crosier@vermont.gov and to Northern Lights for entry into BFIS accounts. Develop a plan to ensure that new employees complete this within 3 months of their start date, and that ongoing employees become recertified prior to their certification expiring.
