Factual basis: Six staff members who have been employed for more than 30 days do not have documentation of completing the Vermont Orientation Training in BFIS, it was not presented on site, and it was not sent to licensing during the week following the site visit. Two of these individuals were present and in ratio during the licensing visit. Corrective action: Ensure that all staff complete the Vermont Orientation Training or an approved on site orientation training prior to being left alone with children or counting in ratio. Send completion certificates to sarah.crosier@vermont.gov and upload them to Northern Lights for entry into BFIS.
