Regulation 5.6.5: All prescription medication administered by staff shall be in its original container, properly labeled for the child, and administered prior to the expiration date on the container.

Factual basis: In the upstairs classroom it was identified that one inhaler was expired and another did not include the prescription box or prescription label. This violation was also cited previously on 5/12/2025. Corrective action: Create a system to identify when medication expires. Also train staff on all requirements when medication is required for children including receiving medication in the original container, having a prescription label and noting the expiration date. Factual basis: There is one staff member who received the Fingerprint Authorization Certificate on 1/12/2026 and they have not gone to be fingerprinted yet. Corrective action: Have the staff make a fingerprinting appointment immediately and send this licensor confirmation of the appointment. Moving forward ensure new staff complete fingerprinting appointments within 60 days of receiving the Fingerprint Authorization Certificate. Factual basis: In the upstairs classroom it was identified that two children had expired rescue medication. One inhaler and one epi pen both expired in January 2025. Corrective action: Create a system to identify when medication expires and ensure that the medication is returned to the family and the family provides new medication.
