    Factual basis: In the upstairs classroom it was identified that one inhaler was expired and another did not include the prescription box or prescription label. This violation was also cited previously on 5/12/2025. Corrective action: Create a system to identify when medication expires. Also train staff on all requirements when medication is required for children including receiving medication in the original container, having a prescription label and noting the expiration date.
