Factual basis: Out of six (6) staff who have been employed for over six (6) months, all six (6) were missing documents in their BFIS accounts. Corrective action: Ensure that all documents are submitted to Northern Lights at CCV. If they are submitted before 5/4/2026 they can be submitted via email with signed cover sheets. If submitted after 5/4/2026, they must be submitted via Direct Send. Email Licensor Rielly when all documentation has been sent. Regulation 5.6.5: All prescription medication administered by staff shall be in its original container, properly labeled for the child, and administered prior to the expiration date on the container.

Factual basis: A child's epi pen was not in the original box properly labeled with the child's name or had the label with the epi pen. Corrective action: Immediately request the box for the epi pen with the label with the child's name and expiration date. Maintain future compliance to only accept medications that are in the original box label with the child's name.
