    Factual basis: The staff have not kept a written record of medication they have given to a child who is required to have medication prior to each meal. The child has been enrolled since September and has had medication daily. Corrective action: Immediately start a medication log to record the time medication is administered, the dosage and if any side effects were observed and save this document in the child's file as required in this rule. Corrective action: By 5/18/2026, all staff must submit the missing training certificates to Northern Lights at CCV through Direct Send and submit the confirmation e-mail to this Licensor once completed.
