The child has been enrolled since September and has had medication daily.    Factual basis: A staff member did not submit a new record check authorization form or submit to fingerprints prior to their expiration date of 3/3/2026. An annual assurance letter was sent to the program, but this staff was not listed in the letter. It is required to have a system to keep track of staff's fingerprint expiration date. Corrective action: Immediately have this staff member submit a new record check authorization form and compete the fingerprint process.    Factual basis: A review of the staff's BFIS quality and credential accounts, staff files, and the support from Northern Lights to find missing documentation, it was determined that five (5) staff members employed longer than six (6) months did not have an up-to-date BFIS Quality and Credential account as required and was missing education documentation, CPR and First Aid certificates and required training certificates. Corrective action: By 5/18/2026, all staff must submit the missing training certificates to Northern Lights at CCV through Direct Send and submit the confirmation e-mail to this Licensor once completed.
