Regulation 5.10.1.1: General Safety: The FCCP shall ensure the physical facilities of the FCCH, grounds, equipment, and toys present no hazard to the health, safety and well-being of the children.

Factual basis: The following was observed on the front porch of the FCCH which is used by children to enter and exit the home: a carpet rolled up and saturated with water, discarded appliances with loose cords, a broken computer monitor, containers with standing water, tools, stacked up plastic panels and small debris such as paint chips and cigarette butts. In the bathroom a large drain snake tool was propped against the sink vanity. In the living room boxes and bins were stacked on top of each other, along the back wall, items are haphazardly stacked up to the windowsill. The stack includes household items, toys, books and papers. Corrective action: The items identified in this report must be removed from the porch and the porch must be cleared of any debris. The bathroom must be cleaned and sanitized and the drain snake will be stored out of the children's reach. There shall be no items stacked on top of each other in the living room. All items that are against the back wall by the window will be removed. Create a program improvement plan that identifies the steps you will take to ensure the interior and exterior of the home remains hazard free. Regulation 5.10.4.1: Access to Useable Space by Children The FCCP shall ensure that interior space designated for use by children is available to children when the FCCH is in operation, is sufficient, and arranged to allow each child adequate space for fr

Factual basis: The space used by the children is cluttered and cramped due to the amount of bins, boxes and toys in the mobile home. The children are left with a very small space in the center of the living room to play. Due to the limited space items are placed in front of the cribs making it difficult to access them. While this Licensor was present the provider had to ask a parent to climb up onto the couch in order to pick their child up out of the crib. Corrective action: All boxes and bins used for storage must be removed. There must be adequate space for the children to move around and they should have easy access to toys and other play equipment. The cribs must be arranged in a manner in which furniture or other items are not placed in front of them. Regulation 5.10.1.10.3: All poisonous or toxic materials shall be inaccessible to children.

Factual basis: The following items were in two (2) red canvas totes on the floor in the living room and were accessible to children, can of fix a flat, two(2) containers of ArmorAll automotive protectant wipes, can of window de-icer and a quart of power steering fluid Corrective action: Immediately remove these items from the living room, ensure that you are removing any poisonous or toxic materials present in the spaces children use prior to their arrival each day. Regulation 5.10.4.5.1: The FCCP shall ensure that the FCCH, its furnishings, and toys are kept in a clean and orderly condition.

Factual basis: Surfaces throughout the spaces used by the children were not clean and orderly. Cobwebs were observed on the ceiling and in the corners of the rooms. Food, dirt and other small debris was noted on carpets and flooring. The stacked up dirty dishes omitted a strong odor as did the garbage can in the kitchen with the dirty lid. Corrective action: Clean all surfaces in the rooms utilized by the children, vacuum carpets and clean the laminate flooring. Review the regulations in section 5.2.6 and create a plan that identifies how you will manage the daily, weekly and monthly cleaning and disinfectant requirements. Regulation 5.2.6.6: The following items shall be Disinfected at the end of each day: * Door handles; * Hand washing sinks and faucets; * Countertops; * Toilets and toilet learning equipment; * Diaper pails; and * Drinking fountains.

Factual basis: The toilet had brown stains, splatters and debris on the seat, rim and inside of the bowl. The area above the waterline had brown particles and stains. The base of the toilet and surrounding floor was also dirty. This is a repeat violation last cited on 07/08/2021. Regulation 5.10.2.1.3: The kitchen and all food preparation, storage and serving areas, and utensils shall be kept clean, sanitary, and free of insects, rodents, dust and other contaminants.

Factual basis: This licensor arrived at the FCCH after lunch had been prepared and served to the children. The provider stated they made hot dogs for lunch. There was a frying pan with grease on the stove surrounded by a stack of dirty pots, a plastic bowl with what appeared to be dried cookie batter. Piles of dirty dishes overflowed from the sink and blocked access to the faucet. Other areas of the counter were cluttered with packaged food, produce, plastic and paper bags. Built up dust and grime were observed on the stove, range hood and other countertop appliances. Corrective action: Immediately clean the counters and stove top. Create a program improvement plan that identifies the steps you will take to ensure the kitchen and food preperation items are kept clean and sanitary. Regulation 5.6.11: The FCCP shall ensure that all medication and non-medications described in the rule 5.6.10 of these regulations shall be securely stored and inaccessible to children.

Factual basis: There was a bottle of Advil pain reliever on a green shelf next to the couch accessible to children. This is a repeat violation from 10/25/2019 Corrective action: Immediately remove the Advil. Factual basis: When I arrived at the home it took about ten (10) minutes for Misty to open the door. I knocked several times and called her cell phone, she stated she did not hear me and when it did it took her a little extra time to get to the door due to a mobility issue. For a majority of time during the visit, Misty remained seated on the couch. She was observed directing the children to put their cots away but did not get up and support. Misty sat in a chair while sweeping food up from the floor. It appeared that her ability to remain standing or moving around may be compromised Corrective action: Please provide written documentation from your primary care physician.
