Regulation 2.3.9.3: The licensee shall ensure that all staff have met ongoing background check requirements by submitting at least once every five (5) years a Records Check Authorization form to the Division and as required shall submit to fingerprinting.
Factual basis: Staff A has not yet completed five-year fingerprints that were due 9/13/2024. Annual assurance letters were sent on 8/20/2023 and 8/19/2024 with reminders to get the prints taken by 9/13/2024. Corrective action: Immediately send a record check authorization form to the Division and schedule fingerprints to be taken as soon as possible. By 3/14/2025, email Licensor Rielly and confirm when prints have been scheduled or, if they have already occurred, email a copy of the receipt. Corrective action: Staff A's BFIS account shows that there were three (3) hours of professional development completed between 10/1/2024 and the time of this report. By 3/14/2025, if training was completed in between 10/1/2023 and 9/30/2024, send the documentation to Northern Lights at CCV and copy Licensor Rielly on the submission. If no training was completed between 10/1/23 and 9/30/2024, email Licensor Rielly a plan that shows how Staff A will complete the missing fifteen (15) hours of annual professional development that was due 10/1/2024 in addition to the twelve (12) hours due 10/1/2025 for a total of twenty-seven (27) hours.
