   Factual basis: Upon review of staff BFIS Quality and Credential accounts and on-site staff files it was identified that four (4) substitutes employed longer than three (3) months do not have current pediatric First Aid and CPR. Corrective action: Immediately have substitutes complete Pediatric First Aid and CPR. Submit certificates to Northern Lights at CCV to be verified and entered into the BFIS accounts.    Regulation 5.2.1.3: The licensee shall ensure the hand washing procedures include the following steps in order: * Use of warm running water and liquid soap; * Moistening hands with water, applying soap, and washing hands for a minimum of twenty (20) secon

   Factual basis: Observed children washing hands before lunch while Staff A stood next to sink. One child was observed washing their hands with water only. One child applied soap, then immediately placed their hands under the water. Another child applied soap and scrubbed for one (1) second before putting hands under the water to rinse. Staff A's attention on the handwashing children was inconsistent. When Staff A was attentive, handwashing procedure improved but continued to lack the proper length of time to wash hands prior to rinsing. Corrective action: Review the handwashing procedure in the order and manner required by this rule with all staff. Develop a monitoring system to ensure compliance moving forward. Send an email to Licensing Field Specialist Stark confirming this requirement has been reviewed with all staff and a detailed description of the monitoring system created.    Factual basis: Upon review of staff BFIS Quality and Credential accounts and on-site staff files it was identified that two (2) substitutes, employed by the program for ten (10) months, have not completed an orientation training approved by the Division. Corrective action: Immediately have both Substitutes complete the Vermont Orientation Training or an approved on-site orientation training. Develop and implement a system to ensure all new hires that may be left alone with children and/or counted in staff/child ratio complete Orientation prior to being left alone with children. Email confirmation of completed trainings and a detailed description of the system developed to Licensing Field Specialist Stark.
