Factual basis: It was discussed that the Provider does not meet qualifications. The Provider was to complete a level one certificate when this violation was cited on 10/12/2020 and has not yet completed the certificate. Corrective action: Reach out to Resource Advisor Joanne Rouleau for guidance and make a plan to complete the level one certificate as soon as possible in order to meet qualifications. Factual basis: The provider called a family asking if their child would be coming to childcare as he was late. The child had arrived at childcare an hour prior to making the call. The provider had forgotten he was there. Corrective action: By 4/16/26, submit a program improvement plan indicating how you will avoid this situation in the future and ensure that all children are accounted for at all times. Regulation 7.1.2.2: The FCCP shall ensure that at least one (1) responsible adult who has successfully completed and is currently certified in pediatric first aid and infant and child CPR is on the premises at all times children are present.

Factual basis: A household member was left alone with the children, while the provider was out of the house, and did not have CPR/First Aid certification. Corrective action: By 4/16/26, have the household member complete CPR/First Aid certification, submit a copy of the certification to Northern Lights and your licensor. Factual basis: The provider's fingerprints have been expired since 10/05/2023. A letter was sent by email to the provider on 01/10/2023 and 01/06/2025 reminding them to complete the five-year renewal check and submit fingerprinting. This is a repeat violation previously cited on 04/02/2024. Corrective action: Immediately complete and submit a background check form. Email licensor to confirm background check has been submitted by end of business day Friday, April 11th. Email licensor with date of scheduled fingerprint appointment no later than Friday, April 25th. Email licensor fingerprint receipt once fingerprint appointment is complete.
