    Factual basis: Staff A did not send in a new Record Check Authorization form prior to their fingerprints expiring in March 2026, and had not sent in the form prior to this licensing visit. Corrective action: Immediately submit the RCA form, and submit for fingerprints as soon as possible once the Fingerprint Authorization Certificate is received.     Factual basis: A condition was placed on the program's new licensed certificate requiring that lead water testing be completed by 1/5/2026. There is no documentation on the VDH website showing that this was completed, the program did not have documentation of this on site, and it was not provided to CDD in the week following the licensing visit. Corrective action: Immediately complete lead water testing. Provide documentation of communication with VDH requesting the testing kits, and notify sarah.crosier@vermont.gov once the testing has been completed and the samples sent back to VDH.
