Regulation 6.2.6.1: Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.
Factual basis: The program director notified the Division that during an off site evacuation drill a three (3) year old child was left alone in their classroom for about five (5) minutes on 10/23/25. The child remained in their classroom, and a staff member found the child sitting in their cubby crying. This is a serious violation which requires notification letters to be sent to all families enrolled at the program at the time of the incident. Corrective action: By 12/12/25, submit your new emergency response plan detailing the steps to all your evacuation drills. The plan should also include how you will be training your current and future staff on the newly developed plan. By 12/12/25 complete the parental notification letter as outlined above.
