Medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Antacid tablets were stored unlocked in a cabinet in the schoolage classroom.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. A spray bottle of sanitizing solution, and a container of baby oil were unlocked in a cabinet in the schoolage classroom.

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present. The current written allergy/sensitivity/diet restriction list in the infant room was dated June 2025 and not the current December 2025 list.

Repeat Violation: Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. The documentation must be signed by a physician , their designee or an official of the health department. The tuberculosis test for staff A and B were not signed by a physician, this information was overdue by 10 months for staff A and by 5 weeks for staff B.

Repeat Violation: Each report shall include the dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. The immunization records for Child A and Child B were not signed by the physician/designee. This information was not provided for 8 months for child B and by several years for child A.

In order to promote the child's physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide needed help, comfort, and support. On 12/4/25 at approximately at 2:30 pm, a two year old child was sitting on their cot during naptime crying. There were no staff in the classroom and the child was not provided the needed comfort, support or help.

Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom provided that: On 12/4/25 at approximately 2:04 pm, staff A left 15- two-year-old children alone in their classroom at naptime. The children were discovered unsupervised by a staff at approximately 2:30 pm.

A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. 1. There was no allergy care plans on file for child A for their nut allergy which was overdue by a few weeks. 2. There was no allergy care plan on file for child D for a diagnosed egg and lentil allergy, this plan is overdue for several months.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. There was no immunization record on file for child B which was overdue by 2.5 months and for child A which was overdue by at least 2 weeks.

The center failed to follow their written playground safety procedures which include active supervision by staff to include positioning of staff in strategic locations, scanning play activities and circulating among children. On "the field" playground there were 3 children, three years of age on one end of the playground playing in a pile of debris which consisted of broken furniture and toys.. The teacher was on the opposite end of the playground and failed to come over to intervene or redirect the children. The staff failed to adequately scan the playground nor circulate amongst all the children.

Orientation training shall be appropriate to the age of the children in care and include all of the following facility specific topics. Orientation training was not documented for staff B, which was overdue by 6 months.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. On "the field" playground there was a pile of debris in one corner, the debris consisted of broken garden furniture, broken toy pieces and lids. This debris was accessible to children on the playground, This debris has been in the corner of the playground since the summer.

Cribs shall be identified for use by a specific child. There were no names of 3 cribs used by infants.

The allergy list maintained in the toddler room was dated 1/24/25 however the most recent allergy list was dated 1/31/25. 2. Child A has an allergy to peanuts as per the allergy care plan, however this was not listed on the allergy list. 3. Child B has an allergy to cherries as per the allergy care plan and this was not listed on the allergy list.

Medication administration shall be performed by a person who has satisfactorily completed a training program for this purpose approved by the Board of Nursing. There was no staff present who had Medication Administration Training for at least 2 hours when children were in care and medications were on site.

The center's procedures for administering medication shall include the duration of the parents authorization for medication, provided that it shall expire or be renewed after 10 work days. Long term prescription drug use may be allowed with written authorization from the child's physician and parent. The parent duration for an emergency medication expired on 9/1/24 and had not been renewed for child A. There was no parental authorization for a non prescription medication for child A.
