Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #2, who had been enrolled for two months.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Staff #1 is currently working and has a TB screening that was dated 2 months after their first day of employment.

Staff #1 who have been actively working at the center for the past 3 months, did not have completed reference verifications on file with date of contact, signature of person who completed the reference check, and the relationship of reference for Staff #1.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #4, who had been attending for 7 months, did not have documentation of an immunization record on file.

Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff # 1 changed Child A and Child B diaper in the two year old room and did not wash the Child A and Child B hands with soap and water.

The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff #1 did not clean the diaper and sanitize the diaper changing surface after chaning after changing Child A and Child B diaper.

The vehicle should meet the safety standards set by the Department of Motor Vehicles and should be kept in satisfactory condition to assure the safety of children. Two vans used for transportation of children inspection expired in December 2024.
