Within the first 90 days of employment, staff shall complete Virginia Preservice Training for Child Care Staff.. Staff B, who has been there for over four months, had no documentation of the completion of the Virginia Preservice Training for Child Care Staff.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. There was no documentation of any immunizations for Child C.

Staff records shall have documentation of education and certification and the experience required by the job description. Staff B had no documentation of education in the record.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days. Medications that are not picked up shall be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. Child G had expired Zyrtec and an Epi Pen that were over a year old. The parents did not pick up the medications within 14 days nor did the center dispose of it as required.

The center must obtain results of a negative tuberculosis (TB) screening from each employee, every two years. Staff # 1 and Staff # 2's most recent TB screenings were last obtained more than two years ago.

The center shall maintain and keep a separate record for each child with completed information. Child F's record did not contain the following information: name and phone number of the child's physician, two emergency contacts names and addresses and phone numbers, and the names of persons authorized to pick up the child.
