The gate leading to the pool pump, where pool cleaning chemicals were being stored, was unlocked and opened while children were on the pool deck.

The merry-go-round was removed leaving a hole in the pour in place resilient surfacing that was covered with a removable mat. The timbers around the area have deteriorating wood creating jagged edges. There are also two steel rods protruding approximately 2 inches above the ground from the timbers. The area is accessible to children in care. 2. The steps of the climber/slide on the pre-school playground are rusted, and the plastic coating is cracked creating sharp edges. The climber is being used by children in care. 3. The pour-in-place resilient surfacing around the climbing boulder and climbing bars have multiple sink holes along with rippling of the surface that poses a risk of altering the effectiveness of the resilient surfacing and being a tripping hazard. Children were observed playing in the area.

The record of staff #3 contained documentation of a TB screening that was completed two days after the date of employment. 3. The record of child #4, enrolled for six months, did not contain the place of employment and work phone number for each parent who has custody. 4. The record of child #5, enrolled for 14 months, did not contain the addresses for the two emergency contacts listed. The record also did not contain annual confirmation of up-to-date information.

The merry-go-round on the school age playground is broken at the axle. 6. Approximately 2 inches of a steal rod is protruding from the timbers around the merry-go-round.

Just before naptime, staff #9 was observed yelling ?Good night?, ?good night? in a harsh tone towards a child who was trying to ask the staff a question. Staff #9 leaned down to the child?s face when repeatedly yelling ?goodnight?. Administration acknowledged hearing staff #9 use the harsh tone.

Child # 9, eight years in age, was observed walking out of one building unattended, across the blacktop area and into the second building.
