Staff #3, who has been employed for over 2 months, had one reference that was completed three days after employment and one reference that was completed 15 days after employment. Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #2 and Staff #3, employed for over two months, did not have documentation to demonstrate that the staff completed the required orientation. Administration stated the orientation was completed but not signed by the staff.

On 8/19/2025, Staff #8 and Staff #10 were supervising 12 children in the toddler classroom during the designated rest period. A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #7 and child #9, who have been enrolled for six months, have a diagnosed food allergy and did not have a written care plan in their file.

Hazardous substances must be in a locked place that prevents access by children. In the younger infant classroom, a cannister of disinfectant wipes was in an unlocked cabinet, out of the reach of children. In a two year old classroom, a bottle of glass cleaner was unlocked on top of a shelf, out of the reach of children. In a second two year old classroom, an unlocked cabinet contained three cannisters of disinfectant wipes, glass cleaner, and sanitizer. The cabinet was not within the reach of children. In a three year old classroom, a container of bleach was in an unlocked cabinet within the reach of children. The bleach contained warning labels to include, "keep of out of reach of children."
