Hand sanitizer was found on a counter in the one a half year classroom and the four to five year old classroom, out of reach of the children.

The record for Child #4, enrolled for one month, was missing the work phone number of one parent who has custody. The record for Child #5, enrolled for one month, was missing the work phone number of one parent who has custody.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 4/27/2026, a three-year old child was unsupervised for approximately 6 minutes.

When staff are supervising children, they shall always ensure their care, protection and guidance. On 4/27/2026 a three-year-old child had fallen asleep in the classroom cozy area and was left alone in the classroom for approximately six minutes after the class transitioned to the playground. No one was in the classroom to offer immediate assistance if needed.

When the outdoor activity area is not adjacent to the center, there shall be at least two staff members on the outdoor activity area whenever one or more children are present. Staff #1 was observed being the only person supervising ten children from the three to five year-old class who had arrived at the playground located across the street, not adjacent to the center. Staff #1 was the only one supervising the children for approximately three minutes.

The required ratio for children ages three years up to school age eligible is one staff per ten children. Staff#1 was supervising eleven children between the ages of three years to school age for sixteen minutes in their classroom.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. The center's employee handbook states "Any employee who has reason to suspect that a child has been abused or neglected must report the suspicion immediately, but no later than twenty-four hours after forming the suspicion. Reports must be made to Child Protective Services ( CPS ) in the appropriate jurisdiction." The center did not follow their policy to report the suspicion of a child being abused within twenty-four hours. CPS was not informed for six days after the center was initially made aware of the allegation.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts. Staff #1 failed to use constructive behavior guidance with two-year old Child #1 on 3/2/2026, when Child #1 put their hand down their soiled diaper, getting feces on the child's hand and then showing it to Staff #2. Staff #1 intervened and moved the child's hand towards the child's face and asked the child, "do you like that?" The soiled hand touched the child's face.

On 8/14/2025, no staff were present to offer immediate intervention and assistance when two children (both three years old) were left alone in the bottom of the stairway for approximately one to two minutes. When Staff #2 heard one of the children crying, they located the children and united them with the class.

On 8/14/2025, Staff #1 was transitioning ten children from their classroom to the music room on the floor below, and left two children (both three years old) unsupervised in the stairway for approximately one to two minutes.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4, #5, and #6, who were all in attendance on the day of inspection.

Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis (TB) screening. Documentation of a negative TB screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. An independent contractor, working with children during the inspection had no documentation of a negative TB screening.

An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds; 2. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and 3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. The records for children #4, #5, and #6, present on the day of inspection, did not have this written agreement.

Chipped paint and pealing wallpaper was found in the 3-years to 4-years classroom. Pealing paint was found in the stairway of the center. A wooden structure on the playground had significant wear and was a risk of splinters. The shed on the playground had 2 broken window panes.
