On February 23, 2026, Child 2 (age 2.5) eloped from an activity room without staff awareness and remained unsupervised for approximately two minutes. During this time, Child 2 was in a hallway and stairwell area not routinely used by the child day program and had access to unsupervised routes leading to the parking lot, restrooms, and offices that are not consistently staffed by childcare personnel.

On February 23, 2026, Staff 1 and Staff 2 were supervising a group of 8 children (aged 2-3 years old). Staff 1 left the group to take Child 1 to the restroom, leaving Staff 2 supervising the remaining 7 children. After the departure of Staff 1, video footage shows Child 2 leaving the room without the knowledge of Staff 2 and out of staff supervision. Child 2 walked down the hallway unobserved and into the stairwell where Staff 3 came upon them approximately 2 minutes after elopement from the room.
