The center's supervision policy requires when children are moving from one place to another, the ?Child Supervision Record (CSR)? is to be used for transitions and to avoid a child being left unattended. On October 9, 2025, Child #1 (2 years old) was let on the playground unsupervised for approximately 6 minutes. Staff #1, Staff #2, and Staff #3 stated they did not complete the CSR when coming in from the playground.

Child #1, (2 years old), was left alone for approximately 6 minutes on the playground while the rest of the class went inside. The staff did not realize the child was missing until their parent came for pick up.

When staff went to look for the child, Child #1 was found inside of an empty classroom that they had entered through an open door from the playground.

During the inspection, Staff #6 changed Child #3's diaper, then changed Child #1's diaper, and did not clean and sanitize the diapering surface after each use. Staff #3 changed Child #4's diaper, then changed Child #12's diaper and did not clean and sanitize the diapering surface after each use.

Repeat Violation The center shall ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. -Classroom A had areas of peeling paint along the window sill. -Classroom B had peeling and chipping paint along the corner of the wall near the windows. -Classroom C had peeling paint on the window sill. -Classroom D had peeling paint on the wall. -Classroom E had glue cracking from a previous fix along the corner of the wall by the back door of the classroom. -Classroom F had large amounts of peeling paint coming off of the children's bathroom door and the metal side coming off of the lockers.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. -Two bottles of glass cleaner and two bottles of disinfectant were in a bottom cabinet with a broken lock under the sink area in the Pre-K classroom. -One bottle of disinfectant and one bottle of restroom cleaner were in an unlocked top cabinet in the Pre-K B restroom. -An unlocked bottom cabinet under the sink in the School Age classroom contained a bottle of glass cleaner, oven and grill cleaner, and a hand soap refill labeled ?keep out of reach of children.? -An unlocked cabinet on the diaper changing table in the Toddler classroom contained surface cleaner, adhesive remover, disinfectant, and restroom cleaner.
