Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Two containers of disinfectant wipes with the warning labels, "Keep out of reach of children," were unlocked in the four and five year old and the two year old classroom, within reach of children. A container of disinfectant wipes with the warning labels, "Keep out of reach of children," were unlocked in on top of the diapering station in the girls bathroom.

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #6, who has been enrolled for over one year, has a diagnosed food allergy and did not have a written care plan in their file.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There were two sink basins in the bathroom showing visible rusting and deterioration along the front edge, with areas of chipped and corroded surface.

When toilet chairs are used, they shall be emptied promptly, cleaned and sanitized after each use. A child-sized toilet chair in one of the bathrooms was observed to have urine in the bowl.
