The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. On page 52 of the Employee Orientation Handbook, it states: "Human milk from a mother shall be used only with that mother's own child." Staff #1 stated that while two (2) identical bottles sat side by side, the label on the given bottle was not checked prior to feeding Child #1.

When staff are supervising children, they shall always ensure their care, protection, and guidance. On 02/25/2026, at approximately 3:30 pm, Child #1 was given the wrong bottle of breast milk. The bottle given was labeled with the name of Child #2.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. In a preschool classroom, a spray bottle of cleaning and sanitizing solution was in an unlocked place, out of reach of children, and was not in use.

The diapering surface shall be used only for diapering or cleaning children. Tables used for children's activities shall not be used for changing diapers. Children activities were observed on top of the diapering surface within one preschool classroom. Staff stated that the diapering table was sometimes in use.

The record for Child #2, enrolled for 11 months, And Child #3, enrolled for 12 months, were missing the following required components: work phone number and place of employment of each parent who has custody.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. The following products were observed on a table inside an adult bathroom that is located inside a bathroom used by children: one bottle of hand sanitizer, on bottle of skin lotion, one odor eliminator spray, one bottle of multipurpose cleaner. The door to the adult bathroom was unlocked and wide open.

When food is brought from home, the following shall apply: The food container shall be sealed and clearly dated and labeled in a way that identifies the owner. In the Infant 1 room there were 4 bottles with milk inside the fridge that had no name and no date on them.
